Communication Cards
For Portage County Residents
Portage County police departments and Mental Health & Recovery Board is offering a service for residents with an intellectual or developmental disability and older adults. The program is to help anyone who feels that having an ID would benefit them in a time of crisis to allow all first responders the ability to understand when interacting with someone on how best to communicate with them and interact with the individual.
Family members can fill out information about their loved one that would be placed on a card to assist first responders with communicating with them in times of crisis. The card gives information such as: name, address, guardian’s name, guardian’s phone number, diagnosis, and best ways to communicate.

This card is meant to assist first responders with communicating, assisting, and making sure care is given to someone with a menatl health crisis, helping people who cannot verbalize, returning a person to home if they wander off, or contacting a guardian if needed.

All information is voluntary and will be kept strictly confidential.
